Patient Forms, Map and Directions to Monroe Office

Your first visit in our practice is an important event. During this visit, we want to develop an understanding of your dental needs and offer our treatment recommendations with your goals in mind.

We begin by spending time getting to know you and understanding your dental health. We would like to know more about:

  • Any current discomfort or dental challenges
  • Your previous dental visits
  • Any changes you would like to make to your smile

This information will help us address your current needs and understand how to best approach your care. We also want to learn about your physical health and make sure that we do everything we can to take extra special care of you.

Our initial examination includes:

  • Necessary digital xrays
  • Gum measurements to check for infection
  • Oral cancer screening
  • Electronic cavity detection to look for early signs of decay
  • Thorough evaluation of your teeth, jaw joints and gums

We work with you to arrive at a treatment plan that meets your scheduling and budget requirements.

Following our in-depth analysis, we review our findings with you to create a plan to keep your smile healthy!

Financial Options
We accept MasterCard, Visa, Discover and American Express. We also participate with CareCredit® and American General health care financing.

Dental Insurance
We work with most traditional dental insurance plans, and ask that patients pay amounts not covered by insurance at the time of service. Read more Facts About Insurance

Since 1981, Dr. Graig Taylor and Team have provided comprehensive dental care to families in Monroe and Walton County, Georgia region.

We never met a stranger…  Our team culture is based on our shared values of family, community service and a lifetime of caring relationships with our patients.  We believe that everyone deserves a beautiful, healthy smile and our commitment to each of our patients is to be genuinely compassionate and supportive in helping them manage their oral health.

Visiting the dentist can be fun!  Each of us has family (and pet) photos to share, as well as a joke or two – we tell all of our patients to arrive early for their appointments, so we can catch up on family news and local events.  And for the little ones, we are always ready to give a “ride in the chair” as part of their first dental experience!
